Proposed state changes would weaken this vital program. Eliminating the Backup Provider Program could leave people without care when a caregiver is unavailable. Ending the Residual Program would cut services if Medi-Cal coverage lapses, even temporarily. Shifting the full cost of care-hour increases to counties could limit services and create unequal access across the state. IHSS is a lifeline for Californians with disabilities and older adults. Weakening the program could force people out of their homes and into costly institutional care.

Over 700,000 Californians rely on In-Home Supportive Services (IHSS) for essential daily activities like bathing, eating, dressing, and mobility. IHSS is a lifeline, allowing people with disabilities and older adults to live safely and independently at home.

California is considering proposals that would weaken IHSS by cutting protections and shifting costs, putting the people who rely on it at risk. These changes are partly driven by the state’s need to offset lost federal funding after HR 1.

We oppose eliminating the IHSS Backup Provider Program and the Residual Program, which ensure care continues when a caregiver is unavailable or Medi-Cal coverage lapses. Without these safeguards, people could lose essential support overnight, threatening their health and safety.

We also oppose proposals to shift the full cost of IHSS care hour increases to counties. This would pressure counties to limit services and create inequities in access across the state.

IHSS prevents unnecessary institutionalization, supports independence, and is a cost-effective alternative to more expensive care. Weakening it would have devastating consequences for Californians with disabilities, older adults, and their families.
